Supplier Representations and Certifications

The Laboratory requires suppliers to complete a Representations and Certifications form along with a copy of an updated W-9 form.
Completed forms may be submitted to the Laboratory by emailing to: repsandcerts@ameslab.gov.
The Laboratory may also request suppliers to complete a Pre-award Form with RFP submissions.
